have been given a copy of the Trinity East United Methodist Church ((The Church) Facility Use Contract and agree to its terms and conditions stated therein. 

I/We understand there will be no consumption of any kind of alcoholic beverages including but not limited to beer, wine, champagne, etc. on Church grounds whether inside or outside.      

I/We also understand this is a nonsmoking facility.

I/We agree to assume all risks of loss, damage or injury, by reason of the condition of the building, grounds, or facilities or by reason of the management control or operation and release the Church, its’ agents, successors, or otherwise thereof.        

I/we agree to hold harmless or indemnify the Church against all claims for such loss, damage or injury sustained by me/us, our agents, members, invitees, or employees or by any person whomsoever arising out of my/our use under this agreement.

I/We understand that no weapons of any kind are permitted inside or outside of Church grounds unless authorized through law enforcement.

I/We understand that due to safety/liability reasons, all minors in attendance must be under the control of adult supervision.

I/We agree that the building, grounds, or facilities to be furnished will be returned in the same condition as received, except for reasonable wear and tear.
